Session Registration Form

Please fill out this form to register for one of our sessions. NOTE: If you are a health authority employee participating from a hospital or PHSA site and wish to participate in a video conference from your site, you MUST fill out a videoconference request form, (located on our 'Connecting' page, and submit it to PHSA. The deadline for submission is 48 hours.
    You may choose more than one session.

Alternatively, You can also contact Jamie Witt at aph.assistant2@familymed.ubc.ca to register for one of our events.
